Registration Checklist

To Reserve Space for your Group the following must be received:

 Parish/Group Registration Form
 $30pp Non-Refundable Deposit Check made payable to: Archdiocese of Denver
(Please submit ONE check (drawn on a US Bank per group)

To Complete Registration the Following Forms must be completed by June 1, 2009:

 Liability Forms for ALL participants
(including Chaperones & Priests)

 Priest information forms

 Chaperone Applications for ALL Chaperones & Priests
(Group leaders must sign all Chaperone Applications)

 Final Payments
