Parish Changes Submission Form

Instructions:

1. Review the current listing on file for your parish(es) as published in the 2011 Catholic Directory. Click here to download the 2011 Directory's Parish Section for reference.

2. If your parish’s information remains the same as what was printed in the 2011 edition, please indicate as such with a checkmark in the designated field below.

3. If updates, additions or deletions are needed to a parish listing, please complete the electronic form below. If any fields are left blank, we will assume that there are either no changes to - or no applicable information for - that particular field. For example, if your parish does not have a website, you can leave that field blank.

4. Please complete a separate form for each parish that is supervised by your pastor. Please pay careful attention to name spellings and phone numbers. If you prefer to complete a hand-written form, click here to download a printer-friendly version. Please follow submission instructions as provided in the document.

SUBMISSION DEADLINE is Thursday, August 11.
